"I established the Dr. John Carey Foundation, with the hope that it could help someone like my dad before it's too late. So many times, when people have a heart attack, there are little to no symptoms leading up to it. It can happen to anyone, even people who appear to be in great health. Though I don't anticipate becoming a physician in the future, I want to carry on his legacy by helping people in the same way he did. As a physician, he saved so many lives. If I could save just one life and prevent another family from experiencing what I have gone through by promoting heart health checks, I know he would be proud of me."

1. Do you have family history of heart disease (heart attack or stroke in a primary relative before age 60?)
2. Do you use tobacco?
3. Do you have high blood pressure?
4. Are you sedentary?
5. Do you get chest discomfort or shortness of breath with exertion?
6. Do you have a BMI greater than 30?
7. Do you routinely experience acid reflux?
